If Eye Innovations is not part of your current medical or vision plan’s network, you may still be able to use us through out-of-network benefits. Check with your medical or vision plan provider to see if you qualify. If you do, simply get a claim form and bring it
us on your next visit . One of our associates will gladly help you fill it out and to provide the necessary documentation.
Flexible Spending Account
If you participate in a Flexible Spending Account (FSA) through your employer, your flexible spending account dollars can be used for eye
exams and prescription eyewear (this includes prescription sunglasses and contact
lenses).
